Controlled studies of tonsillectomy in children with periodic fever syndrome (PFAPA) have not been reported, but some retrospective studies suggest that a substantial number of fever episodes have ceased after tonsillectomy. A boy diagnosed with PFAPA is presented. The boy had a history of recurrent fever every third week from one year of age. A tonsillectomy was performed when he was 3.5 years old. The episodes of fever ceased immediately and have not reoccurred 16 months after the operation. We suggest that tonsillectomy should be considered in the most persistent and troublesome cases of PFAPA. Paediatricians and otolaryngologists should collaborate on the diagnosis and treatment.
